We are writing to you today with an important message to all suppliers who may visit areas where invasive procedures take place across all hospitals in Wales.

Any representative accessing Theatre Departments or Day Surgery Units in Wales must have the following uploaded to the credentials section of their MIA profile before attending site:

· A DBS certificate issued within the last three years

· Evidence of Hepatitis B inoculation (if you have not completed your immunisation course, please upload evidence of the jabs you have received so far. We can also accept a serology test to show proof of immunity).

These requirements must be met before attending any clinical area where invasive procedures take place.

Please note that if these requirements have not been met, your appointment may be cancelled. Failure to comply with these credentialing requirements could result significant consequences - disruption and additional costs for the Health Board, difficulties for the representative and clinical team, and, most importantly, an avoidable impact on the patient. Please review your MIA profile now and upload any missing or expired documentation before arranging or attending visits within these areas.

If your documentation is already uploaded and in date, no further action is required.
